Nasal Pyramid
Operation Technique
Dr. Bessler explains the operations technique "Push-Down-Let-Down". He mostly uses this technique, to straighten a hump nose.
Hump Nose 1
This young woman was dissatisfied with the hump on her nose. She felt that her face was too harsh and wanted softer features.
During the procedure, I removed the hump, shortened the nasal tip and corrected the internal structures of the nose. In this way, I was able to shape her nose more harmoniously and improve her breathing in the process.
Hump Nose 2
The nasal pyramid runs from eye level to nasal tip. Deformation of the nasal septum is often clearly visible and obstructs nasal breathing. The specialist world differentiates between four different types of nasal pyramid: Hump nose, crooked nose, saddle nose, and hooked nose. All four possible shapes are illustrated here with examples.
The nasal pyramid on this hump nose curves forwards. This curve, as well as the length of her nose displeased this young woman. In addition, her sleep was disturbed by obstructed nasal breathing. After meticulous planning, I straightened the nasal pyramid with a nose operation and raised the position of the nasal tip slightly to suit the new nasal pyramid. The right-hand pictures were taken 3 months after the operation.
By straightening the nasal pyramid and raising the nasal tip slightly, I was able to significantly improve the patient's nasal breathing.
I stabilised the nasal tip in its new position. In this way, the nasal pyramid also remains straight when she laughs.
As the images were taken 3 months after the procedure, the swelling in the nose has not completely gone down yet. A narrowing of the nose will become evident over the next 6 months.
Hump Nose 3
This young man was unhappy with the hump on his nose. I corrected the hump in the upper area of the nasal pyramid, and the drooping nasal tip. The nasal tip is now aesthetically aligned with the nasal pyramid.
Before the operation, the nasal tip dropped when the patient laughed, which made the hump stand out even more. After the operation, the nasal tip and the nasal pyramid remain aligned with one another even if the patient laughs.
Thanks to the narrowing and straightening of the nasal septum and the new position of the nasal tip, I succeeded in lending the face an overall harmonious appearance.
Hump Nose 4
This man had difficulties breathing through his nose due to his crooked nasal septum. He felt uncomfortable with the crookedness of his nose in the frontal view. From a specialist perspective, the imbalance between the narrow nasal pyramid and the broad asymmetry of the nasal tip disrupts the harmony of the face. The images were taken 12 months after surgery.
I straightened the nasal pyramid profile and moved the nasal pyramid to the middle line of the face. I also improved the patient's nasal breathing, even with a significantly narrower nasal tip. By balancing the nasal pyramid and the nasal tip, the nose becomes more elegant and the face more expressive. The three-quarter view gives a clear picture of the crooked nasal pyramid before the operation.
I also reduced the angle between the nasal columella and the upper lip. This changes shape and contours of the nose as a result.
Hump Nose 5
This young lady was also dissatisfied with the hump on her nose, which is clearly visible in the side view. I smoothed out the nasal pyramid; however, I didn't change the height of the nasal tip. Without a nasal hump, which was removed during the surgery, the patient's face looks far more femininely and slender.
Hump Nose 6
This young lady felt her nose was too big and out of proportion with her face. She was disturbed by her nasal hump and the drooping nasal tip.
With the nose operation, I was able to lower the nasal pyramid and narrow down the nasal tip. The after pictures were taken a year after the operation.
During the operation, I stabilised the nasal tip. Now it doesn't retract any more when the patient laughs, but instead remains above the level of the nasal pyramid.
During the nose operation, I had to make an incision through the nasal columella (see the red arrow). The residual scar is almost invisible.

Crooked Nose 1
When someone has a crooked nose, the nasal pyramid points to the side or is curved in itself. This often significantly obstructs the person's breathing.
This man could hardly breathe through his crooked nose. In addition, he was unhappy with the shape of his nose, which had an aesthetic impact on his entire face.
I straightened the nasal pyramid and, in the process, also harmonised the remaining parts of the nose. The man's face looks friendly and balanced now.
The pictures were taken a year after the nose operation.
The strong degree of asymmetry is very evident in the half-crooked view from the front. This caused the nose to have two different shapes, depending on the side you looked at it.
When he lowered his head, the crooked nasal pyramid was clearly visible.
Crooked Nose 2
This patient suffered from severely obstructed nasal breathing with snoring and restless sleep as a result of a kink in the nasal pyramid.
During this challenging operation, the bony parts of the nasal pyramid had to be widened, the inner structures straightened and stabilised with tissue from the patient's own body. I succeeded through meticulous planning and execution. The patient's nasal breathing was freed and he stopped snoring as a consequence. Latter is rather uncommon in similar procedures.
In aesthetic terms, the shape of the nose is more attractive. Although a complete mirror symmetry was not achieved, the nasal pyramid looks much more balanced now.
Crooked Nose 3
This lady was unhappy with her crooked nose and the asymmetry of her nostrils. In addition, she could only breathe insufficiently through the left side of her nose.
I straightened the nose significantly. This is clearly evident in the area of the nasal pyramid and the nasal tip. Even the base of the nose has symmetrical nostrils after the operation. In some straightening operations, a slight asymmetry of the nasal pyramid is not preventable.
In the view from the right, the nose and the nasal hump seem bigger. This is due to the crooked alignment of the nose. The jointly determined goal of the nose operation was to straighten the nose from the front and from the side, without changing the character of the patient's face in the process.
The severe deviation in the nasal septum had led to obstructed nasal breathing on the left side. In addition to the aesthetic change, I also greatly improved the patient's nasal breathing.
Crooked Nose 4
This young lady did not like her nose for several reasons. In her opinion, it looked crooked when viewed from the front. In addition, she was disturbed by the hump and the drooping nasal tip in the profile picture.
She wanted to make a major change to the shape of her nose.
Together we simulated various new nose shapes on the computer and decided on this new look from among all the feasible solutions.
I straightened the crooked parts of the nose under surgery, set the nasal tip back and raised it. This is how the new stub nose shape came about; it makes the patient's face look more harmonious and delicate.
Crooked Nose 5
I like to show this picture because it reveals a further delicate aspect of cosmetic nasal surgery: light reflection. During every operation, I need to keep an eye on the results from different perspectives. Even a small area which throws the light back in another way can strongly influence the overall picture of the shape of a nose. This is why all nasal structures need to be taken into account.
Crooked Nose 6
During a bicycle accident, this man broke his nose and suffered injury to the skin. Besides hardly being able to breathe through his nose, he was disturbed by how the scar was pulling inwards and by the irregularity of the nasal tip.
During the operation, I realigned the nasal pyramid and the nasal tip, and freed up his nasal breathing. The comparative pictures were taken a year after the operation.
In addition to the width and the S shape of the nasal pyramid, I was also able to correct the irregularities in the area of the nasal tip.
Saddle Nose 1
If a nasal pyramid has collapsed, this is known as a saddle nose. A saddle nose often comes about as a result of an accident or a badly performed nose operation.
This client had previously been operated by another surgeon. As a result, his nasal pyramid had dropped, which also led to a deterioration in his nasal breathing.
The after pictures were taken 18 months after the nose operation.
During the operation, I straightened the nasal pyramid and rebuilt it with tissue from the patient's own body. As a result, it freed up his nasal breathing. In this case, I could have slightly lengthened the nose downwards, which I had recommended for aesthetic reasons. However, the patient decided to forego this part of the operation.
Saddle Nose 2
This young man had broken his nose when he was a child, which resulted in a major growth disturbance. The nasal tip collapsed during the growth period and the patient developed a hump on his nose. The man also complained about obstructed nasal breathing.
I formed the nose with tissue from the patient's own body and restored his nasal breathing ability. The position of the nasal tip and the shape of the nasal pyramid were also rebuilt with tissue from the patient's own body.
As a result of breaking his nose as a child, the tip had dropped and the nasal wing had widened. By straightening the nasal tip, the nasal wing narrowed in size.
Saddle Nose 3
This man fell on his face several years ago without being able to protect himself with his arms. Since then, the tip of his nose had pointed downwards and he had had difficulty breathing through his nose.
I was able to straighten the saddle nose and raise the nasal columella. As a result, both his nasal breathing and also the aesthetic appearance of his entire face normalized.
Hooked Nose 1
The hooked nose is similar in shape to the hump nose. However, the curvature of the nasal pyramid is emphasised by the sunken position of the nasal tip.
The lady depicted was uncomfortable with the profile of her nose. She felt as though she had a hawk nose. Her hooked nose generated a strongly rounded nose profile with a deep nasal pyramid at eye level.
During the procedure, I straightened the nasal pyramid by raising it to eye level with natural tissue and lowering the other parts. As a result, the nose looks much more balanced. In this process, all incisions could be made inside the nose.
Hooked Nose 2
This young woman felt uncomfortable with her hooked nose. The drooping nasal tip and the angular tilt disturbed her as well. The after pictures were taken 14 months after the operation.
The operation was more difficult than the straight result perhaps suggests. I had to loosen all the existing nasal structures and put them back together to achieve the desired results.
The straightening of the nasal pyramid and the new position of the nasal tip are clearly visible here.
I raised the nasal tip markedly. This is most evident in the profile picture.
Now the nasal tip no longer drops when the patient laughs.
I was also able to correct the angular tilt of the nasal pyramid.
Hooked Nose 3
In the case of this young lady, I raised her nasal tip slightly and straightened the hook on the nasal pyramid. Her entire face relaxed and harmonised itself, which is clearly recognisable in the area of the nasal wings. They also seem to be perfectly proportioned now.
Hooked Nose 4
This young woman not only wanted to be able to breathe better but also to have a delicate, feminine-shaped nose. The after pictures were taken 6 months after the operation.
In the side view, the reduction in the size of the nose is visible. The nasal tip is sitting higher.
Before the operation, the nasal tip was pulled down when the patient laughed, which disturbed her.
It becomes clear in the front view: not only the shape of the nose has changed, the nasal tip is now in a different position.
Large Hooked Nose 1
This young woman thought her nose was too dominant. She didn't like the nasal hump and also had severe problems breathing through her nose; she had to breathe through her mouth at night and her sleep was restless.
It was very important for the patient that the character of her face was retained where possible. I took this into account during all steps of the operation. The young woman is pleased about being able to breathe through her nose without difficulty. The after pictures were taken a year after the operation.
In the front view, the nasal pyramid and the tip of the nose are in closer alignment with one another, and the nostrils are more balanced.
Following the operation, the nasal tip remains in place, even when the patient laughs.
The suggested slight lift to the nasal pyramid was not performed at the explicit wish of the patient in order to retain the character of her face.
Large Hooked Nose 2
This patient did not like the size of her nose with the forward curvature of the nasal pyramid. I corrected the shape of the nasal pyramid and the position of the columella.
The computer simulation in the picture below shows the planned operation result. The original contours are also presented in the left-hand profile picture. The computer simulation can never correspond to the actual operation result – I always advice all my patients in advance.
During the operation, I straightened the nasal pyramid and left a slight curve in it. The tip of the nose was refined and moved upwards slightly. The nasal wings also had to be refined; an incision was required along the nasal wings to achieve this. The after pictures were taken 2 years after the operation.
The curvature of the nasal pyramid to the patient's right side could be straightened with the nose operation.
The drooping nasal columella and the crookedness of the nasal pyramid were corrected by the nose operation.
By straightening the nasal pyramid, refining the nasal wings and the nasal tip, the nose appears more elegant and the patient's face more harmonious.
