Why is the revision rate of aesthetic nose surgery higher than in other aesthetic surgical procedures?
Rhinoplasty is a complex operation. The following six factors must be taken into consideration when performing rhinoplasty:
1. Internal and external nose shape
In any nose surgery, internal and external nose shapes must be considered, which increases the complexity of rhinoplasty surgery.
- The inner nasal shape: The complex three-dimensional surface of the internal nasal cavity is essential for physiological nasal breathing. We need physiological nasal breathing for oxygenation of the body, especially the brain, and for a healthy and restful sleep. Therefore, the inner form, which means the nasal cavity, is considered in priority in any nasal surgery.
- The external shape of the nose: The outer three-dimensional shape of the nose contributes significantly to facial aesthetics. The nose surgery is based on the desired shape of the nose.
2. Anatomy influencing nasal breathing
Nasal breathing is influenced by many other factors that must be considered during nasal surgery:
- The swelling behavior of the turbinates.
- The collapse tendency of the nasal entrance during inhalation.
- The texture and moisture of the mucous membranes.
3. Stability of the inner and outer shape of the nose
In shape-reducing nasal surgery, the stability of the nose should not be neglected when the cartilage structure is removed. When the skin of the nose thins out with the aging of the body, the nose must not collapse.
4. Shape of outer and inner nose skin
The nose is composed of 3 layers: The inner and outer nasal skin and the cartilage and bone layer. The latter's shape is changed during rhinoplasty, while the outer nasal skin still has the shape of the original nose. The inner nasal skin can be changed to a limited extent. The inner and outer nasal skin must be able to conform to the new cartilage and bone shape of the nose, without bulging due to its elasticity.
5. Elasticity of the nose
Parts of the nose are elastic due to the movable cartilage. During facial movements and touches, the tip of the nose also moves. During nose surgery, unlike the bony jaw, the shape of the nose cannot be fixed in position and shape with screws. Therefore, the elasticity of the nose determines nose surgery.
6. Changes in the shape of the nose after nose surgery
The nose shape changes significantly 12 months after nose surgery. These changes after rhinoplasty must be calculated and may contribute to the need for overcorrection.
